I think the threads on each end should be opposite of each other, other wise when adjusting you would just be moving the sleve back and forth, kinda like your describing.
86 heads are junk your better off going with a set of e7te's you should be able to find some on ebay for $50 plus shipping. I know that your just doing this as a repair but I dont have a full answer to your question so I am only stating my opinion.

If you car is a 5 speed and you plan on staying naturally aspirated I would go with 3.73's If you plan on going turbo later I would just get 3.55's. Im not a big fan of 4.10's unless you have an auto.
Take off the starter and the battery and take them to autozone to be tested for FREE. Rule out your problems with out dumping money into parts you dont need.
