throw out bearing

1966 200ci. 3 speed on floor. the throw out bearing keep popping of the throw out arm. All new parts and nothing was forced when assembled.

could the arm pivot be bent enough to make the arm disconnect?

Loose/worn engine trans mounts or worn z rod pivots can cause this.

pulled the trans and bell housing and found the arm pivot point is off about 3/4 inch which pulls it off the bearing. do I have the wrong bell housing or wrong arm?

What do you mean by off 3/4". IRRC the release fork has a spring clip that keeps it snug to the pivot ball in the bell. If it is broken, bent, or not holding it allows the release fork to flop around and fall off of the TO bearing. If you can wiggle the release fork in the bell and it's sloppy enough to contact any edges of the window in the bell the fork is not seated on the pivot ball. I'm assuming everything was fine before the clutch was replaced and all the new parts were the same.

Check the input bearing retainer on the trans (the collar around the input shaft)

It might be worn and letting then T/O brg shift sideways enough to come off the fork?
