the easiest way is to get a decent kit that has everything you need. the next best way it to get the brackets from someone like https://www.mustangsteve.com/product-category/rear-disc-brake-conversion-brackets/ or https://scarebird.com/index.php?route=product/category&path=65 the mustang steve page has just rear disc kits, the scarebird page has both from disc kits and rear disc kits listed.

understand that these are just the brackets, you still need to get the indicated calipers, rotors, and other parts.
