What are some good first mods for the 4.6l Mustangs. I heard an offroad X and Steeda Timing Adjuster really helps. I was going to pick up a Short Thorw anyways (looking at MGW) and I've also heard good things about JLT CAIs and 4:10s.

If you are doing gears and timing adjuster, I would recommend a handheld tuner. SCT and Predator are popular choices. I have a predator and it adjusted the speedo for my gears, you can adjust the timing with it, you can turn off the MIL that will come on with your O/R midpipe and there is some other cool stuff you can do.
Other than that, I agree with what you have listed. Just as long as you know, once you get started modding, there is no stopping.
I say go with bang for the buck. Gears don't fear the gear got 4.10's lots of fun. Timing adjuster will be on by this weekend, wasn't alot of bang for the buck but now I got a bunch of people wanting a shot n/a style soo. Exhaust OR pipes are nice best advice try and find a sound you like and get that. Also I personally not a huge fan of tuners early off but if your doing gears, TA, O/R, and other things new prices I think it is way cheaper.
What results will I see with a tuner? I really don't want to put premium in my car and I know some tunes require this. Could I still see good gains without upping the octane?
I think you could retard the timing with the predator tune and avoid premium gas, but that would probably take out some or most of the performance gain from it. You listed a timing adjuster in your original post and you do know that if you use that you will have to run premium also. That's the reason I said to try a tuner was because it will help with some of the mods you have listed, gears for speedo, O/R mid pipe-turns off MIL's, and replaces the need for a timing adjuster.
