Quarter window handle keeps popping off

My rear quarter window handle has a set screw and it continues to fall off. I have no idea how to fix it so that it stays. Maybe I need to put more pressure on it when I set it. Any tips or tricks ?

I used a Dremel tool and made a flat spot on the window crank (not the handle) for the set screw. I put a little thread lock on the set screw. Never had another issue with the handle falling off.
