Ticking in 1st?

When I leave off in 1st gear, my car makes a loud ticking noise. It's only in first gear, never when the car idles or in any other gears. It needs a tune up/oil change BAD and has an exhaust leak. But if it was skipping, wouldn't it tick all the time? Anyone know what would cause it to tick only in first?

My cars transmission just took a dump, and I am having the tick/rattling sound you might be referring to. My engine rebuild mechanic seems to think its the input shaft bearing bouncing around. That bearing, I am thinkin the bearing is only accessed through the gear box, in other words might as well have a total rebuild.
Might ticking against the bearing retainer.
