98 gt cutting out bad! HELP!!

Car idles fine until i start to drive it and it cuts out and bucks. It stutters alot when under a load and full throttle until around 4900 rpms then it stops and opens up but not up to full power. It sounds like crap to. It was throwing 2 codes 1151 and 1152. any help is appriciated.

Codes P1151 and P1152.
Air intake system leaking.
Vacuum hoses leaking or damaged.
Air leaks after MAF sensor.
02 Sensors damaged or failed.(Possible one or more 02 sensors failed.)
Possible air leaks at the PCV valve or at related hoses.

change the plugs and go chase that vaccum issue.. mine broke up so bad when i got water in the plugs that it sounded like a 2 step..
