I have access to all the equipment needed for the fabrication. I'm just looking for a template from someone that has already built their own. Or if someone could measure their bought unit I'd appreciate it.

pro link sales a upper control arm set up for pretty cheap you might want to check it out before making your own

If you're interested... I made my own double adjustable uppers and lowers but no spring perch as I'm using a relocated coil-over setup.
I have pictures of my build here;

I used chromemoly tubing and Aurora rod ends with NAS bolts.
