I think you got most of the big stuff. A few things I can think of to add are

Bellhousing
Clutch fork
Upper and lower shifter boots
Dust shield
And a couple small things like the pilot bearing and correct speedo gear

It would be a good idea to get a firewall adjuster and quadrant, I didn't and wish I did. MM has a kit that comes with an OEM cable, adjuster and quadrant for about $121, here's a link

http://www.maximummotorsports.com/cq.asp#packages

Also if the clutch fork pivot stud in the bellhousing is worn you will probably want to replace it.

Good luck

I forgot to add that the driveshafts are the same length. You can use the AOD driveshaft, just be sure that the balancer clears the tailshaft seal on the tranny.

If you do need a clutch fork and clutch cable, buyfordracing.com sells a kit that comes with both for $40

The driveshafts are similar, but from what I understand the AOD shaft is not splined as long as the T5 shaft is and this can cause some issues later down the road. I did take an AOD shaft out of a T5 car before I knew this, and it seemed a little messed up -- so I kind of have to agree with it. You can use a T5 shaft in an auto car and be fine, but an Auto shaft in a T5 car will case problems eventually. The FMS shaft works in both. I'd probably go for an FMS piece and be done with it.

What you need to do the swap:
flywheel, clutch, pressure plate, tranny with bellhousing, clutch fork, pedals, wiring harness, speedo gear, exhaust hanger, shifter, clutch cable, quadrant, pilot bearing. The bellhousing bolts are different lengths for the t5, and I think that the flywheel bolts might be different too. Also the console plate for the shifter might not be a bad idea. I've heard of guys having issues with the AOD tranny crossmember but I can't verify as I simply swapped in a t5 unit from a donor car. Don't forget plugs for the tranny lines in the radiator, you won't be using that anymore. Good luck.

Are the flywheel bolts different from the Automatic and the T-5?

Yes, the bolts are different lengths. The flywheel requires a longer bolt than the flexplate.
