I PM'd droptopford 5 0, and asked him the same question. Here's his answer:

i actually had tried to look up the info quickly but, being the idiot i am i misplaced the reciept with the info on it and it was not with all of my other mustang parts and labor reciepts. However, I had looked again and found it in another place so here it goes... aside from all of the gaskets, fluid, filter etc that were replaced... he replaced my converter with a heavy duty lock-up torque converter, replaced pump, (all seals, bushings, and outer o-ring), and also replaced 2-4 accumulator assembly (updated) w/ electronics. I know the new accumulators are highly recommended by ford as they basically knew they screwed up big time with the original ones they used. The main other problem here that would cause me to hit neutral or another gear while in OD or basically any other gear was a fluid line pressure leak with one of the seals and the converter was fried. Hopefully this will give you a few things to look into. I'm not saying this is exactly what is wrong with your car, since i only know the basics of our aod-e's and am not the one who serviced mine. But just make sure all your line pressure is ample with no leaks, upgrade the accumulators either way and check into the converter. Hopefully this will give you a direction for you or a local trans shop to look into. Good luck - oh BTW, all of this after parts, labor, and fluid was just under $1200, so maybe after all you might consider a whole new rebuilt trans. It's up to you.
