Axle Swap

I have a 91 5.0 LX with the 2.73 gears. Is it possible to put the axle from an 03-04 GT on this cars. That way i would have 3.27 gears and rear disc. Are the axle connection points the same, drive shaft lenght, what kind of brake mods required for the rear disc?

I know i would have to convert the front wheels to 5 lug.
