Engine Codes... Runs

I went and pulled codes on my car today and found codes.
181 - Fuel system at rich adaptive limit at partial throttle / system lean
186 - Fault in injector pulse width circuit / high
189 - Fuel system at rich adaptive limit at partial throttle / system lean

Not sure why the two codes are the same. I'm thinking this may be a Maf issue but I need some other thoughts before I go changing parts.

The car is sluggish if given full throttle and sometimes at part throttle. It also reaks of fuel often.

Before you change the mass air take the element out and make sure it is clean. If you see any grey or black on it then use a q-tip and alcohol to gently clean it off. They are notorious for causing driveability issues like you mentioned.
