I'd guess they were tightened to the point of stripping the threads out of the crank. There may be other reasons for that though. If there were harmonic issues, or imbalance problems, They may have been rattled out of the crank.

Are you sure they threads have been removed and its nut just thread locker on the bolts? The iron crank should be a lot stronger then the bolts so I would expect the bolts to get messed up first.
