HELP: clutch will not disengage!

Ok we have a new T5, new clutch, new PP, TOB, resurfaced flywheel, etc. on a 5.0

Engine runs fine, clutch FEELS fine, but we cannot put the car in gear with the motor running and clutch in.

I tried starting the motor with the car in gear and the clutch pedal in, and it launched the car. The pressure plate is still in full force on the disc.

We thought whatever it is, the trans has to come back out. We thought maybe somehow the disc was backwards or the TOB was put in wrong, but we took out the gearbox and everthing looks fine. The TOB is pressing on the clutch fingers but the clutch disc is not releasing. Has anybody ever had a similar problem?
 
also wanted to note that we shimmed the cable like crazy. i pulled it out and put a hose clamp in there to ensure we were getting full cable travel. with the shim, it was partially disengaging the clutch. we are stumped!
 
stock quadrant? when i did my clutch and all that stuff after i was done i lifted up a little on the clutch pedal and stepped down and heard a clunk and now it works fine. Id give that a try
 
we pulled the clutch cable so far out that it was partially disengaging the pressure plate. i yanked on the cable while my friend unscrewed the firewall adjuster.

also, we just jacked the car up in the back, started it in 5th gear, wound it out to about 100mph and dropped the car with the clutch pushed in and it just smoked the tires.
