Pulsing/ Jerking In drivetrain under Accel..

Out of curiosity, does a fuel pump gradually start to fail or is it an off/on kind of thing? Also, does a failing pump make a lot of noise?

Mine gradually gave up the ghost. Didn't hear anything though. Just hesitation and bucking at high RPMs.
