This is my flywheel after 13miles of use, and it was reserfaced 13miles ago. is it me or its actually looks like was never machined flat???

It could have been that your clutch was sucking hind teat and wasn't meeting the flywheel; more likely, from the way it looks.

i tell you what... this whole change clutch thing is turning in to a nightmare....
Blown clutch, then replacement clutch was POS, and then TOB obviously failed after 13miles, and now this.....

Let's trade cars, I don't mind swapping in a whole new clutch & flywheel

Seriously, it does suck. I had to go through all of that about 5 months after I first bought my stang, so I know how much of a pain in the ass it can be. What Clutch did you get?
