What can cause a t/o bearing to go bad over and over

I have a 03 gt and my t/o bearing went bad so I dropped the tranny and went ahead and replaced the clutch , pilot , t/o bearing put everything back together and I had some seriously bad vibration in the clutch pedal. I checked it to me not knowing what I was doing so I took it to my local tranny shop and he said the throw out bearing was bad again , so my question is what could be causing my car to eat t/o bearings?

How often to you "adjust" the clutch pedal by pulling up on it?

How often to do set at lights with the clutch pedal held in?

Who made the throw out bearing? Good maker or off shore?

Do you know if the throw out bearing was lubed and how before initial install?
 
A new OEM Cable with an aluminum quadrant and firewall adjuster would be a helpful upgrade too, first thing I do. Stick with OEM for the most part if available.
