I have a 91 LX with the 5.0L and AOD. Yesterday, i got stranded not too far from my home, where it appears the AOD shift linkage grommet (with attached threaded rod and nut) on the cable popped out of its hole on transmission. Is this pretty common and what's the best source for the grommet? are they pretty easy to press in?

I am not very AOD savey more of a manual gear box kind of guy. I am sure a picture would help here as someone with more AOD knowledge will swing by shortly. Maybe @Mcmahst or @JD1964 knows something here.

I've done brand x auto grommets , they are pretty easy to press in , I used channel locks.

You have an internal problem inside of the transmission for that assembly to fall out. Watch this video as it clearly will show you what you need to know.

Never mind my post above. I now see you are dealing only with the outside portion of the shift lever
