Accel cap & rotor problem

Last night I was getting on my car a little bit. Went through first and second pretty hard and as I was shifting into third...huge backfire. The car shuts down and I roll to a stop.
I try to start it and it just rolls over. Check the fuel pressure..perfect. Checking the new plug wires..nothing there. Checking my new Accel cap & rotor and there it is; the metal piece on the rotor was off and mangled.

Has anyone else experienced this? Scared the crap out of me.
 
Just the cap and rotor, or do you have the entire Acell dizzy? I noticed with my Acell dizzy that it was clocked wrong, I had to move all my wires over one lug to make it fit.
 
I did that on another car once. Same kinda deal. The cool thing was that the spark was still making it across the gap even though the timing was off (since the metal was all mangled up). When idling, inside the car sounded like a vacuum cleaner with something wrapped around the drum (general crappy noise).

In my case, it was a one time thing so I chalked it up to a parts issue. Had it happened again, I'd have pulled the dizzy to check mainshaft play.
