Blew apart my pilot bearing!

I've been having trouble getting my car into gear every once in a while (into ANY gear), so I figured it had to be something messed up in and around the clutch area. Sure enough, the pilot bearing was blown to peices! Most of the needle bearings are crushed to dust, and the outer ring is cracked into a bunch of pieces. My question is.....what could make this happen? One thing I want to know is during installation, should I push it back as far as it will go into the crank, or just leave it flush with the out side of it? I'm not sure what I did last time, but want to make sure I do it right this time! Thanks!
