Aside from the normal squeeks and rattles everythings fine driving down the road. When I hit the breaks I hear a knocking noise coming from the rear. It sounds as though it knocks relative to the speed of the wheels, so could this be something in the drums? Rear end gears/axles (although I don't hear it unless I'm on the breaks)? I bought new shoes, but should I get new drums too?

I'm kinda hoping it's the gears, it probably isn't, but that will give me a reason to finally get my 3:73's installed.
 
I would check U-joints. The inertia of the forward motion vs the opposite (braking) force can load and unload the joints.

Otherwise, make sure the shoes are wearing even. I have had a shoe get cockeyed and wear one corner as the braking occurred. The shoe was being pulled downward and it would make noise as it loaded up.

There are a ton of other things, but some quick checking should narrow down if it is something like these things or something deeper in the rear, etc.
