Overheating!

I have a 1995 5.0 manual and its overheating, put coolant, but don't know much about cars. To be honest, I'm not sure why it's overheating. I can't really afford to take it into a mechanic. I know enough that I could probably get by if it isn't something major like a head gasket. Any advice?
 
Maybe take some pictures or upload a video of what is going on to YouTube and post a link. You need to provide as much information as you possibly can. We are not there so seeing pictures and videos really help.

Other questions:
Is the car stock? If not what has been done if you know.
How many miles?
Does it have the stock radiator, fan, and fan clutch?
Is the serpentine belt in good shape?
Are there any leaks?
Where does the stock temperature gauge read normally and when you are having problems?

You see where I’m going with this?
 
Is the car new to you such that it already had this problem before you got it? Or was it working normally in your experience and then later began overheating?
