#2 Intake valve problems

I have had the head worked on by two different machine shops in the past year. After a few months I bent the # 2 intake valve. I replaced the cam and lifters, moved the rockers and pushrods around (Don't hassle me on that) but I still get a bent valve stem on the #2 intake. I need ideas. Thanks
 
Changed everything when I replaced the cam this last time. I do not have push rod guides. Early 289 4bbl head with non-rail rockers. I asked the last machine shop to measure everything and they said nothing was wrong on the head so the cam and valve train were replaced. I am beginning to wonder if I did have a problem and when the first valve bent the stem guide was wallowed. But I specifically asked to have that checked and to re-machine with an oversize stem valve if necessary.
 
sounds sorta like valve guide to valve seat alignment. i had some major issues with this after i had hardened valve seats put in. first off i had a valve get stuck from scoring the stem, this happend 4 times!! so i just had the machinist ream the guides to .345 fixed the stuck valve problem. something else i noticed with this, it sounds like i have lifter tick when hot not cold. no amount of adjusting can cure it. last time i had the heads off i could let gravity seat the valve(no spring). then pust it down and it would move sideways about .003 or so, and feels a little stuck when unseating them.
