Alright, so here is what my mustang is doing. a more updated descriptive version)
We ran it through this many times.
Either under load, or under high acceleration, on the 1-2 upshift, the mustang starts to shift in to second, than shifts back down into first skyrocketing the rpms, and if you stay on it eventually goes back into second.

But heres the catch, you have to do it from a stand still and the car has to be idling for a few seconds. We think it might be the 1-2 accumulator piston/housing causing the problem, and as the guy in my last post suggested.
 
Does it shudder/shimmy? Still sounds like accumulator, being from a stand still might just be the line pressure not having proper build up time and is not able to sustain for the shift due to the worn accumulator. I'm really not sure there, but i know those symptoms.
