help! faulty throw-out bearing

I own a 2000 mustang gt with a t-45 tranny. Recently i had a king cobra clutch kit installed and after about 2500 miles the throw-out bearing started making noise. My mechanic replaced it and also installed an frpp clutch quadrant and firewall adjuster. He adjusted it so the throw-out bearing is never spinning unless the clutch pedal is depressed, yet within two weeks the bearing was making noise again. I suspected the bearing retainer might be the cause, but it's a steel retainer and he said there was no sign of galling or distortion. The only explaination he can think of is that possibly they had a bad batch of bearings that didn't get properly greased or something. If anyone has had the same issue or knows a possible cause, any suggestions would be greatly appreciated.
 
Make sure the clutch is aligned properly. Just saw a guy with a relatively new center force at the shop the other day. He didnt have is clutch aligned properly, after 5k you could see the wear on the pressure plate and the throw out bearing was hosed.
 
Since I bought my stang (also a 2000), I have replaced the TOB twice. The car had 62,000 on it when I bought it, has like 95k now.

The first one was put in 2 weeks after I bought the car, and that one went bad in approx 5k miles - it started chirping. I dealt with it for a looong time, never really had too many issues other than the noise, but then the clutch pedal feel got bad. I tried putting in a FRPP adjustable quadrant/cable and it helped with pedal feel, but the noise didnt go away. Put a new TOB in, and it's been good so far.

TOB's go in these cars left and right. It really just seems to be the question of when.
