95 OBD1 Codes

Hallo everyone im new to this site, and in search for help. I recently bought a 95 5.0 Gt all stock with a couple of problems with it When i try to accelerate hard on wide open throttle, it feels like its not getting gas, like it bogs a bit and the check engine light comes on. So i bought an OBD1 code reader and got these codes

on KOEO
CODE: 181,189, fuel system at rich adaptive limit at partial throttle / system lean

KOER
CODE: 172 HEGO (HO2S) Sensor Fault / lean
CODE: 213 Spout Circuit Open

On code 213 (Spout Circuit Open) i found out that one of the tooth from the spout cap was bent and was not inserted in right. Fixed that problem, and that code is gone. It Accelerates a bit faster, but it still feels like im losing power. What can be causing these problems?
 
Clean the MAF sensor if you haven't.

Clear the codes and re-run them after a couple of drives. You might have a vac leak, MAF issue, or O2's that have seen better days. On 94-95s, bad O2's lead to some real driveability issues.

After that... it may be worth the time and realtively minor expense, of getting your fuel injectors cleaned and flow tested. Rich at part throttle while lean on top leads me the idea that injectors may be clogged or that the spray pattern is lousy.

If you do get them tested, be sure to buy an o-ring kit for reinstallation. Some silicone lube would be nice too.
