Bad Shifter Rattle

My car has a B&M shifter installed by the previous owner. It was quiet for the first few months, but it has developed an irritating rattle from vibration above 2,500 rpm. I pulled up the console and found that the four allen bolts on the shifter were really loose. So I put some thread lock on the bolts and it was quiet for about six weeks (except for 3rd above 2,500) and now it has returned. Any ideas?
 
There is a nylon bushing/insert in the receiver where the shifter goes into the transmission. If that is worn or gone, it could cause a rattle. Just put the shifter in neutral, unbolt the 4 bolt on the top of the shifter plate , pull up the shifter and see if that insert/bushihng is there (either on the end of the shifter or in the receiver in the transmission).
 
I put in a new bushing and didn't really help. The bushing that was in the car looked almost new. When parked and I move the shifter, all the gears have very little slop; except for 3rd and that's when it rattles the worst. I guess I'll just have to live with it.
