Car troubles please help...

My '95 mustang has a jerky acceleration when it warms up and car shakes when I'm sitting at red light. When I start the car it runs ok but it gets worse when the car warms up. I replaced spark plugs, put in injector cleaner into gas but that didn't help. Does anybody have any ideas?

I had a problem not to long ago with my V6 about the same thing. Like two weeks ago it just died at the light. The other day my and my friend were sitting at a stop light and the car drop to 400 rpm I hit the gas to raise it again and it was fine. Try raising you idle rpm it should clear it up for you. Also check your throtle cable.
 
Try cleaning the IAC. Just pull off the intake and spray some carb cleaner into the IAC which should be connected to the throttle body. Should be some small holes that lead to it. Usually, if it has a problem idling and the solution is to press the gas slightly, it's usually the Idle Air Control.
