collapsed lifter?

Over a year ago I put in the e303 cam with new lifters and just recently I had a sound coming from the passenger valve cover. We pulled it and found to what looked like a collapsed lifter. They were under warranty so I put brand new ones back in started it up and the sound was still there . Is there something that could be making them collapse? I have stock length gt pushrods and rockers on gt40 heads. Also my oil pressure was inconsistent while this was happening...oil pump finally going out maybe?

You may have a blocked oil passage going to the lifters. It could also be physical damage to a cam lobe. Start it up with the valve covers off and check how consistent the oil flow is out of each rocker.

that was my next step to pull the valve covers. i was adjusting my timing when it started and for a couple weeks before hand my dummy oil pressure gauge would drop to 0 when my rpms would drop past 1000. if that helps w/ anything?
