Most likely your tank isn't venting when you fill up. I had a bronco that did the same thing because someone took the vent hose out of the filler neck. Maybe yours is gone too? Or maybe it's just kinked... Fix that, and I'm sure you won't have any more problems filling up. On a side note, I could get mine to work at full pressure at *some* pumps by cocking the nozzle to one side. Good luck man!
I just skimmed throught the post so I hope I'm not repeating anything...but there's a drain hole in the little metal dish between the filler neck and the outside body of the car. Take an airhose and blow it down in there..you'll see it. It often get's clogged with dirt and you never know it's there. That'll keep fuel from overflowing out of that spot and should prevent some of it from running down the outside.
As for the fuel just spewing everywhere...The only time I've ever seen that is when the nozzle is pushed TOO far into the filler neck. Good luck figuring that out though. I've never had a problem on mine. Plus stations down here in Georgia have big rubber circles that block any backspray.
