Identify this intake

Saw this intake for sale on Craigslist around my area and can't say I have ever seen it before. Any clue what it is?

Weird. It's like an old Downs Ford/Cartech intake, but for a truck lower.

Exactly. I almost scrolled past it thinking it was Cartech but then saw it's shape and those ports.
