Lug nuts keep coming loose on one wheel

3rd time now. Always the same wheel (front driver side). The first 2 times, the wheel had been put on by my mechanic. The last time, by me, and I VERY carefully torqued them to spec, following a star pattern.

What could be causing this?

Pull the wheel and look at the back side where it mounts up against the brake rotor. Careful inspection may reveal some fine cracks radiating from the bolt holes.
