Where does the pvc hose from the rear go?

It goes to one of the large vacuum ports on the intake manifold. Intake vacuum should "pull" on the crankcase through the PCV (positive crankcase ventilation) valve. Be sure you provide a way for air to enter the crankcase - that's what the hose between the valve cover oil filler neck nipple and throttle body is for. If you're replacing stuff, it's a good idea to put in a new rubber grommet for the valve to assure no vacuum leaks there. Also, there is a metal screen below the grommet that traps oil and minimizes oil in the intake. THat should be replaced as well. If they're clogged, you can build crankcase pressure which manifests itself with seals and gaskets leaking oil.
 
I have a pvc valve in the oil filler neck that goes into the throttle body. the large port on the intake manifold (only one on the Performance Products intake) goes to the vacuum tree I thought, for brake booster vacuum. The PVC on the manifold rear is just open to the world at this point. Should that go to the intake manifold also? That would give me two PVC valves.

Alan - there should be no pcv (pvc is poly vinyl chloride) valve between valve cover and throttle body - just a hose. The pcv valve should insert into a rubber grommet in a large hole in the back of the lower intake. Below the valve should be a metal screen that acts as an oil trap. A rubber hose on the top of the pcv valve should connect to a matching size vacuum port on the intake manifold - full vacuum should pull on the pcv valve in the lower intake. Sounds like someone who didn't know what they were doing plumbed yours up incorrectly.
