I'm having a similar issue mine doesnt sound or act like it's running lean only signal is the code. Mine's not got a Vortech though just a Bbk intake/throttle body. I've done most of the same tests you have minus a smoke test.

For the plugs that didn’t look like they were working, was that only on one particular bank?
Could be COPs, injectors…I’d change the plugs first since they’re cheaper
Make a new thread, and state exactly what you did. I’ll check it out and I’m sure a new thread will have more eyes, vs my thread that has a title with “resolved”
